- Provide physical therapy services to patients according to a written individualized plan of care.
- Assess and evaluate therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
- Evaluate home environment, muscle strength, mobility, gait, range of motion, and transfer capabilities.
- Improve or minimize residual physician disabilities of the patient.
- Provide physical therapy as prescribed by a physician or advanced practice practitioner.
- Educate patients on equipment required to increase function and independence, returning them to an optimal and productive level within their capability.
- Possesses a degree from an accredited master’s or doctoral program in physical therapy.
- Preferred 1 year of clinical experience as a physical therapist.
- Strong interpersonal skills and ability to communicate professionally with staff, patients, and caregivers.
- Knowledge of medical terminology.
- Ability to demonstrate problem‑solving, conflict resolution, organizational skills, and attention to detail.
- Strong verbal and written communication skills.
- Ability to handle multiple priorities, documentation requirements, and deadlines.
- Reliable transportation and valid automobile insurance coverage.
- Ability to sit, stand, bend, lift, and move intermittently.
- Ability to lift at least 35 lbs and bear the weight of an average adult effectively.
- Home health experience preferred.
- Familiarity with Home Care Home Base (HCHB).

$45—$50 USD
