• Provide physical therapy services to patients according to a written individualized plan of care.
• Assess and evaluate therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
• Evaluate home environment, muscle strength, mobility, gait, range of motion, and transfer capabilities.
• Improve or minimize residual physician disabilities of the patient.
• Provide physical therapy as prescribed by a physician or advanced practice practitioner.
• Educate patients on equipment required to increase function and independence, returning them to an optimal and productive level within their capability.

• Possesses a degree from an accredited master’s or doctoral program in physical therapy.
• Preferred 1 year of clinical experience as a physical therapist.
• Strong interpersonal skills and ability to communicate professionally with staff, patients, and caregivers.
• Knowledge of medical terminology.
• Ability to demonstrate problem‑solving, conflict resolution, organizational skills, and attention to detail.
• Strong verbal and written communication skills.
• Ability to handle multiple priorities, documentation requirements, and deadlines.
• Reliable transportation and valid automobile insurance coverage.
• Ability to sit, stand, bend, lift, and move intermittently.
• Ability to lift at least 35 lbs and bear the weight of an average adult effectively.

• Home health experience preferred.
• Familiarity with Home Care Home Base (HCHB).

$45—$50 USD
