The Physical Therapist administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician or advanced practice practitioner’s orders and plan of care under the direction and supervision of the Home Health leadership team.

• Provide physical therapy services to patients according to a written individualized plan of care
• Assess and evaluate therapeutic, rehabilitative and functional status, and participation in the development of the total plan of care. Evaluate home environment
• Assess muscle strength, mobility, gait, ROM and transfer capabilities. Improve or minimize residual physician disabilities of the patient
• Provide physical therapy as prescribed by physician or advanced practice practitioner
• Educate on equipment required to increase patient function and independence, returning the individual to optimal and productive level within the patient’s capability

• Possesses a degree from an accredited Master’s or doctoral program as a Physical Therapist
• 1-year clinical experience as a Physical Therapist preferred
• Interpersonal skills and ability to communicate professionally and effectively with staff, patients, and caregivers
• Knowledge of medical terminology required
• Problem‑solving and conflict‑resolution skills, organizational skills, and attention to detail
• Strong verbal and written communication skills
• Ability to handle multiple priorities, documentation requirements and deadlines
• Reliable transportation and valid automobile insurance coverage
• Can sit, stand, bend, lift, and move intermittently
• Can lift at least 35 lbs and bear the weight of an average adult effectively

• Home Health experience preferred
• Familiarity with Home Care Home Base (HCHB)

Pay Range: $45 - $50 USD
