- Evaluate patient’s functional status including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.
- Evaluate home environment for hazards and barriers to more independent living.
- Participate in the development of the individualized plan of care including equipment needs.
- Develop treatment program and establish goals for improved function.
- Communicate plan of care to referring physician and other organization professionals.
- Teach new skills or retrain patients in once familiar daily activities that have been lost due to illness or injury, in accordance with the agency's policy.
- Maintain appropriate clinical records, progress notes, and reports to the physician or advanced practice practitioner regarding any changes in the patient’s condition. Submit records in accordance with the agency's policy.
- A graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
- 1-year clinical experience as an Occupational Therapist preferred.
- Registered with the National Registration Examination of the American Occupational Therapy Association.
- Requires interpersonal skills and ability to communicate professionally and effectively with staff, patients, and caregivers.
- Knowledge of medical terminology required.
- Able to demonstrate problem-solving and conflict resolution skills, organizational skills, and attention to detail.
- Must have strong verbal and written communication skills.
- Demonstrates ability to handle multiple priorities, documentation requirements and deadlines.
- Reliable transportation and valid automobile insurance coverage.
- Position requires sitting, standing, bending, lifting, and moving intermittently.
- Ability to lift at least 35 lbs and bear the weight of an average adult effectively.
- Home Health experience preferred.
- Familiarity with Home Care Home Base (HCHB).

Marion, Hamilton, Madison, Hancock, Shelby, Johnson, Morgan, Henricks, Boone, and Clinton.
- Office
Hours:
8 AM - 5 PM, Monday - Friday. - Per Diem.
- Weekend Rotation.
Pay Range: $29–$31 USD.
