What are the job responsibilities?

The Registered Nurse (RN) Case Manager will provide comprehensive care coordination and support to hospice patients and their families. The RN Case Manager will be responsible for assessing patient needs, developing individualized care plans, coordinating care services, and providing emotional support throughout the end-of-life journey.

Key responsibilities include:
  • Clinical Assessment and Care Planning
  • Symptom Management and Pain Control
  • Emotional Support and Counseling
  • Care Coordination and Collaboration
  • Documentation and Compliance
  • On-call Rotation for after-hours and weekends
What do you need?
  • RN licensure for the state of Indiana
  • Clean driving record, and valid driver’s license and auto insurance
