• Provide 1:1 care to make a lasting impact on patients and families

• Assess and monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Administer medications, treatments, and interventions as part of direct nursing care
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with the interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by the local branch

• Graduate of an accredited nursing school with current licensure to practice in the state of operation
• One year nursing experience required; 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an insured automobile in accordance with state or organizational requirements, in good working order
