Community Staff Nurse - Milnthorpe

Milnthorpe District Nursing Team (now based at Yealand Conyers) and the Community, Cancer, Diagnostics & End of Life Division (CCDE) has an exciting opportunity to recruit to a full-time permanent Band 5 community staff nurse position. We would like an enthusiastic and highly motivated Band 5 Community Nurse who will be happy to work in an evolving district nursing service. The successful applicant will be committed to working closely with health and social care colleagues to provide high quality care to patients with a wide range of medical conditions in the community setting.
Applicants must hold a full UK driving licence.
The role requires excellent communication skills as you will be undertaking complex nursing assessments and developing and delivering programmes of care for patients and carers. The district nurses work a 24‑hour service and there is an expectation that you may be required to support this to ensure service delivery. You will need to work collaboratively with other professionals, voluntary sector and informal carers to promote excellent care to patients in their own homes and community setting.
You will be joining an enthusiastic and motivated team who are friendly and approachable. We are very patient focused and staff development is high on our agenda. As part of our team we want to develop all staff to the highest standard. You will need a full UK drivers licence and access to a car in order to fulfil this role.
We operate from three main hospitals – Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal – as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant‑led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non‑life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, daycase and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Person Specification Qualifications- Registered Nurse (on the relevant part of the register for the post)
- Mentoring and assessing qualification or working towards or equivalent
- ECDL or equivalent
- Post registration clinical course appropriate to area or willingness to undertake further training relevant to the post – and to use and maintain these skills in the delivery of care
- Evidence of recent professional development
- Experience of establishing effective communication
- Working within a multidisciplinary team
- Experience of implementing change
- Previous experience within designated area
- Experience of mentoring and assessing members of the team
- Experience as shift team leader
- Specific clinical skills appropriate to speciality or willingness to undertake
- Ability to prioritise own workload and that of others
- IT skills – use of email, word processing
- Ability to communicate effectively both written & verbal
- Knowledge of Clinical Governance
- Flexible working will be required to meet the needs of the patient group and the service
- Ability to travel to provide assessment and treatment to patients in the community and attend meetings
