APP – Community Palliative Care

Overview

UChicago Medicine Advent Health Bolingbrook, Glen Oaks, Hinsdale and La Grange hospitals are affiliated with UChicago Medicine, one of the top academic medical centers in the country. We are also part of Advent Health, a unified health system with over 50 hospital campuses and hundreds of care sites in diverse markets throughout 9 states.

UChicago Medicine Advent Health is hiring for their Community Palliative Care program. This is a growing program which is so needed in our communities. The program serves our homebound patients, giving them access to palliative care in the comfort of their homes.

• 1-3 years of APP experience, preferred
• Palliative Care Certification, preferred
• Weekly Monday - Friday schedule
• No call requirement
• Outpatient palliative care in patient’s homes
• Visit patients in assigned territory
• Work collaboratively with Social Workers and House Calls Physicians & APP's
• Competitive compensation, benefits, CME and occurrence-based malpractice insurance
• Annual Salary: $110K - $120K

REQUIRED QUALIFICATIONS:

Education/Training

• Graduate of an accredited school of nursing and nurse practitioner program
• Masters degree in Nursing or Physician Assistant Studies
• Current Illinois license in good standing
• Certified as a nurse practitioner or physician assistant by an approved certifying organization.
• Prescriptive authority, Illinois Controlled Substance, DEA certification
• Current CPR certification
