First Health of the Carolinas is a nationally recognized health care system located in central North Carolina. Comprised of four hospitals (Moore, Hoke, Richmond, and Montgomery) with more than 600 beds, the system also offers leading-edge heart care in the Reid Heart Center, the area’s only dedicated heart and vascular center. As of March 2023, First Health of the Carolinas officially opened its new comprehensive cancer center to patients.

Home Care RNJob Summary
- The RN Case Manager is responsible for the clinical management of an assigned patient population.
- The RN Case Manager develops a patient centered, comprehensive plan of care utilizing a multidisciplinary team approach.
- The RN Case Manager is accountable for achieving positive patient outcomes
- Supervises Home Health Aides and LPNs according to policy
- Reports daily to the Clinical Manager all pertinent patient information
- Reports/records information accurately
- Uses appropriate safety and infection control measures
- Graduate of an accredited school of nursing with a current North Carolina RN license or RN multi-state licensure privilege through the Nurse Licensure Compact.
- Minimum of one (1) year of acute care or home care experience required. Evidence of current CPR certification required.
- Valid North Carolina Driver’s License and proof of auto liability insurance required.
