The RN case manager will work collectively with members of the interdisciplinary team to facilitate and oversee a care plan and symptom management needs in the home environment. The role provides flexibility in delivering skilled complex care to hospiceيب patients and families in the community while educating them on advancedunnut disease processes.

• Graduate of an approved school of nursing with a current NC RN license or compact state RN license.
• At least two years of experience in an acute care facility or home care agency.
• Must possess a valid NC driver’s license.
• National Certification (CHPN) recommended after completion of at least two years of full‑time experience as an RN in a hospice nursing practice.
