Contract osition Description edigence Health provides Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace members/patients of primary care physicians, medical groups, and health plans we serve. The Physician/Nurse Practitioner, will perform comprehensive assessments which are compliant with the annual wellness visit, in a home setting, in a clinic setting, via telemedicine or in long term facility. Responsibilities: omplete a detailed evaluation of the medical and social assessment of the beneficiary members. dentify and refer members/patients for routine, urgent, and emergent case management. ake recommendations for members follow-up with their primary care physician. ravel throughout local counties, regions, and/or states where you hold a current license. omplete comprehensive assessment in provider office or Telehealth visit supporting Audio and Video connectivity.

bility to document within available EMR. urrent unrestricted license in the state of Connecticut. UST live in Connecticut ot required to prescribe or alter the members current treatment plans. Required

Skills and Experience:

years hands on experience as a Nurse Practitioner or Physician. nowledge of HCC model, HEDIS and STAR quality measures urrent, unrestricted license in the state or the ability to acquire one ust be registered with Medicare. ust be board certified, preferably internal medicine, geriatric or family medicine specialty ffectively communicate with elderly and chronically ill members omfortable working in a Provider Office/Clinic and using telehealth technology roficient in use of computer, tablet, and mobile technology- #   Galveston, TXCritical Need  Children’s is seeking a part-time Night Nurse Practitioner (NP) to join our growing inpatient and PICU team at Shriners- #   Farmers Branch, TX  do it all with heart, each and every day.
