RN Case Manager – Home Health

We are looking for an experienced Registered Nurse (RN) Case Manager to join our growing team in Carson

• Provide skilled, one‑on‑one nursing care in patients’ homes.
• Complete thorough OASIS assessments and design individualized care plans.
• Monitor progress, document accurately, and coordinate with the interdisciplinary team.
• Educate patients and families on care, medications, and self‑management strategies.
• Collaborate with physicians, therapists, and aides for seamless care delivery.
• Participate in case conferences and quality improvement initiatives.

• Active RN license in Nevada
• Minimum 4 years total nursing experience
• At least 2 years in a Medicare‑certified home health agency
• Proficient in OASIS assessments and Medicare documentation
• Strong clinical judgment, communication, and organizational skills
• Reliable transportation with valid driver’s license and current auto insurance
• Familiarity with EMR systems like Axxess (preferred)
