Overview

LPN (Healthcare)

Responsibilities
  • Administrative Functions: Direct the day to day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long term care facility. Ensure compliance with facility policies and procedures. Review department policies, manuals, and job descriptions; make recommendations for revisions. Plan shifts, services, programs, and activities with assigned staff. Ensure Nursing Service Procedures Manual is current and reflects day to day nursing procedures.

    Make written and oral reports/recommendations concerning shift activities. Cooperate with other resident services to maintain residents’ total regimen of care. Ensure personnel comply with respective job descriptions and nursing service procedures. Participate in quality assurance and facility surveys as requested. Review and update discharge plans; assist in planning discharge components as necessary. Interpret policies and procedures to personnel, residents, visitors, and government agencies.

    Admit, transfer, and discharge residents as required. Complete accident/incident reports as necessary. Maintain the Daily Census Report and related administrative tasks. Complete administrative duties such as medical forms, reports, evaluations, and charting. Maintain confidentiality of user IDs/passwords and resident information; report violations as required.
  • Charting and Documentation: Complete and file required records at admission, transfer, and discharge. Encourage physician review of treatment plans and orders. Receive and transcribe physician orders; document nurse notes clearly. Complete accident/incident reports; report to Director as required. Manage dietary orders and coordinate with Food Services. Record diet changes, transfer forms, and ensure proper documentation of unauthorized discharges.
  • Drug Administration Functions: Prepare and administer medications as ordered; verify resident identity before administration. Maintain adequate stock; order medications and supplies as needed. Ensure narcotic records are accurate for the shift and report discrepancies. Review medication cards for accuracy and adherence to stop orders. Dispose of drugs per procedures and notify the physician of automatic stop orders.
  • Personnel Functions: Participate in staffing discussions and evaluations; recommend staffing adjustments. Provide leadership to nursing personnel; perform daily rounds to ensure standards. Ensure CNAs are enrolled in approved programs; supervise nurse aides and trainees. Foster a positive, collaborative work environment and ensure adherence to policies and procedures. Manage complaints and coordinate with Nurse Supervisor for problem resolution. Ensure staff follow safety, dress code, and hand hygiene policies.

    Maintain good interdepartmental relationships to support resident care.
  • Nursing Care Functions: Inform staff of new admissions and room assignments; prepare rooms for new residents. Escort and orient new residents/families; participate in physician rounds as needed. Arrange diagnostic/therapeutic services; consult with physicians regarding care and treatment. Review care plans and ensure nursing staff follow them; supervise direct care by licensed staff or trainees as appropriate. Coordinate restorative and rehabilitative programs;

    inspect treatment areas daily. Administer professional nursing procedures and use restraints only per policy. Collect specimens, take vitals, monitor seriously ill residents, and ensure dietary considerations align with care plans. Notify physicians of incidents or changes in condition; arrange transportation for discharges; assist with post-discharge arrangements and funeral notices as requested.
