• Assess home health and hospice patients to identify physical, psychosocial, and environmental needs and document findings.
• Develop, implement, document, and evaluate the Plan of Care to ensure quality and continuity of care.
• Educate patients and families regarding care, medications, safety, and self-care practices.
• Supervise and provide clinical directions to HHA/STNA/LVN staff to ensure quality service.
• Perform on-call services and respond to patient needs appropriately.

• Remote, hybrid, or on-site options based on schedule notes.
• Schedule:
  
  28 units per week; possible on-call responsibilities.
• Compliance with infection control and safety standards (OSHA).
• Effective communication with patients, families, team members, and healthcare professionals.

• Current RN licensure in the state of Nevada.
• CPR certification.
• Graduate of an accredited nursing program (Associate Degree or Nursing Diploma);
  Bachelor's preferred.
• Homecare Homebase and OASIS experience required.
• CHPN certification preferred.
• Valid driver's license and reliable transportation.
