• Conduct Home Health Nursing Assessments to determine nursing equipment needs and develop care plans with the IDT.
• Deliver and document home health nursing interventions as outlined in care plans, including maintaining a safe environment, responding promptly to physician orders, administering medications, and performing ordered tests.
• Educate participants, families, and caregivers about disease processes, self‑care techniques, and prevention strategies.
• Track and monitor Home Health scheduling with the Home Health Manager or scheduling point of contact.
• Collaborate with the Welbe Health Physician and IDT members to facilitate smooth care transitions between hospitals, skilled nursing facilities, home, and other settings.

• Graduate of an accredited School of Nursing.
• Unencumbered RN license required.
• Minimum of 1 year of nursing experience in a clinical setting with a frail or elderly population; home‑health agency experience preferred.
• Reliable means of transportation.

Salary/Wage: $47.74–$63.02 per hour plus bonus and equity. Actual pay will be adjusted based on experience and qualifications.
