Registered Nurse - Full Time (Carson, CA)

Prudential Health Care is seeking a dedicated Registered Nurse to join our Home Health team located in Cerritos, serving the Orange County area. This full‑time position offers competitive hourly pay ranging from $45 to $50 per hour and the opportunity to make a meaningful impact on the lives of senior adults and families throughout the region.

• Provide nursing care through all phases of care, taking into account age and developmental needs.
• Collaborate with and support the plan of care as determined by the physician and the healthcare team.
• Treat patients according to approved protocols and physician orders.
• Perform periodic re‑evaluation of patients as necessary and adjust the treatment program accordingly.
• Utilize OASIS assessment and documentation procedures.
• Facilitate timely discharge based on goals in accordance with client policies and procedures.
• Educate and instruct patients, families, and staff as required.
• Counsel patients and families to meet nursing and related needs.
• Supervise and train LVN’s, HHA’s as required for patient care according to law and regulations.
• Document all care, intervention and coordination per law, regulation and agency policy.
• Notify the patient’s physician and responsible staff of significant changes in a timely manner.
• Coordinate all services provided; consult with other health professionals as needed.
• Effectively plan, organize, set priorities and complete work assignments with minimum direct supervision per agency policies.
• Assist Supplemental Health Care in determining skill and specialties.
• Comply with institutional policies and procedures.
• Communicate with Supplemental Health Care regarding problems and concerns.

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
• One (1) year’s nursing experience, preferred.
• Acceptance of the philosophy and goals of this agency.
• Ability to exercise initiative and independent judgment.
• OASIS experience preferred but not required.
