A Medical Social Worker (MSW) provides social work services to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and the plan of care under the direction and supervision of the Director of Clinical Services/Nursing Supervisor. Services are furnished by a qualified social worker or by a qualified social work assistant under the supervision of a qualified social worker.

• A master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education, and one year of social work experience in a health care setting.
• Or a baccalaureate degree in social work, psychology, sociology, or a related field, and at least one year of social work experience in a health care setting (functions as an assistant).
• Two (2) years of appropriate experience for an assistant position.
• One (1) year of experience in home care, preferred.
• Acceptance of the philosophy and goals of the Agency.
• Ability to exercise initiative and independent judgment.
• Ability to work with individuals to enlist cooperation of many people to achieve a common goal.

• Understand and adhere to established Agency policies/procedures.
• Understand and promote principles of QAPI.
• Act as a consultant to other Agency personnel.
• Participate in the coordination of the Agency’s services with community resources; use community resources.
• Observe confidentiality and safeguard all patient information.
• Serve as a resource person to employees, patients, physicians and other allied healthcare providers.
• Develop a cooperative relationship and communicate effectively and professionally with physicians.
• Accept responsibility for regular attendance and punctuality.
• Maintain current knowledge of Medicare Conditions of Participation for social work services.
• Immediately report any accident, incident, lost article or unusual occurrence to the Director of Clinical Services/Nursing Supervisor.
• Participate in in services and/or continuing education programs.
• Participate in patient care conferences.
• Maintain contact with community support groups and provide professional expertise as required.
• Plan/coordinate all social services within the Agency.
• Document all patient/family services provided as required by Agency policy.
• Assist the physician and other team members in understanding the significant social and emotional factors related to health problems.
• Participate in the development of the plan of care.
• Prepare clinical and progress notes.
• Work with the family.
• Participate in discharge planning.
• Perform other duties assigned by the Director of Clinical Services/Nursing Supervisor (or Social Worker if Social Work Assistant).

Works indoors in the Agency office and at patient homes, traveling to/from patient homes.

Director of Clinical Services/Nursing Supervisor. If Social Work Assistant:
Social Worker.

Ability to participate in physical activity and to bend, lift, and/or stand on a regular basis; light lifting may be required; ability to work for extended periods while sitting or standing.
