Schedule:
Monday through Friday - can be very flexible with times and hours but need 40 hours
Pay Rate: $16-$19/hr depending on experience and includes full time benefits such as Paid Time Off, Paid Holidays, 401k, Medical, Dental, Vision, etc.
REQUIREMENT:
Bilingual English/Spanish preferred and 1-2 years experience in patient administrative care required

Job DescriptionThe Patient Care Manager (PCM) position completes the client case history (intakes) for all sites per assigned location, schedules new client evaluations, manages conversion meetings, monitors referrals and admits daily, ensures patients complete initial visits, as well as maintains the scheduling information about therapists on staff, such as availability and areas of expertise.
Team ResponsibilityTreat fellow administrative team members, clients and therapists with respect and politeness. To come to work on time and follow established dress code policies.
Operational Responsibility- Coordination with physicians for attainment of required documentation (i.e. diagnosis paperwork, testing, etc.)
- Coordination with Business Development Managers as required.
- Attendance of evaluations and rescheduling when applicable.
- Confirmation calls for initial evaluations.
- Educating parents on the importance of therapy and adhering to the plan of care.
- Coordination of cold cases.
- Carry out any other projects as assigned by the Director of Revenue Cycle and Systems Management and Division Director.
- Be able to effectively communicate unique selling points of Cole Health to potential referral sources and patients/family members in order to receive patient referrals to Cole Health.
- Can organize, analyze, record, disseminate, and maintain applicable reports, logs, and profiles related to sales and marketing of Cole Health.
