Overview
Join to apply for the Home Health Speech Therapist role at PHC Home Health in Raleigh/Durham, NC. The Home Health Speech Therapist provides medically prescribed speech therapy services to patients in their homes under the direction of the physician. You will work with patients who have communication and swallowing disorders, as well as cognitive and language impairments. Develop treatment plans in collaboration with the treating physician, monitor progress, and adjust treatment as needed.
Educate patients and their families on speech therapy techniques and exercises.
- Evaluation and functional assessment of the patient and establishment of a plan of care.
- Provide therapeutic techniques for the rehabilitation of patients.
- Coordinate care, including care management and discharge planning as needed with the patient/responsible party, physician, other disciplines and external resources.
- Document accurately, objectively and timely in accordance with regulations and agency policy/procedures.
- Master degree in Speech-Language Pathology
- Current state license to practice as a Speech-Language Pathologist
- Minimum of 1 year of experience in home health or related setting
- Experience working with patients with communication and swallowing disorders, cognitive and language impairments
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Valid driver license and reliable transportation
- Assessment and evaluation of speech and language disorders
- Development of treatment plans
- Implementation of speech therapy techniques and exercises
- Monitoring and adjusting treatment plans as needed
- Documentation and record-keeping
- Education of patients and families on speech therapy techniques and exercises
- Collaboration with other healthcare professionals
