Cary / Raleigh / Durham, NC

PHC Home Health is a rapidly growing full‑service Home Health Agency seeking a skilled and compassionate Licensed Practical Nurse (LPN) to provide nursing care to patients in their homes, ensuring comfort and safety under physician direction. Candidates should demonstrate excellent communication, time‑management, and a strong commitment to patient care.

Responsibilities
  • Provide nursing care to patients in their homes, including treatments, diagnostic procedures, and patient education based on the plan of care and physician’s orders.
  • Administer medications and treatments as prescribed by the physician.
  • Monitor patients’ vital signs and report any changes to the physician.
  • Fulfill duties outlined in the patients’ care plans and suggest changes to the supervisor and physician as needed.
  • Observe signs, symptoms, treatment reactions, drug interactions, and changes in the patient’s condition; provide appropriate follow‑up and reporting.
  • Coordinate care with patients, families, physicians, and other disciplines or resources.
  • Communicate with patients’ families and caregivers.
  • Maintain accurate, up‑to‑date patient records.
  • Collaborate with other healthcare professionals to ensure optimal patient care.
  • Participate in multidisciplinary team meetings.
  • Participate in learning and job‑enhancement activities as directed by the supervisor.
  • Participate in on‑call rotation.
Requirements
  • Current LPN license in the state of North Carolina.
  • Minimum of one year of experience in home health or a related field.
  • Excellent communication and interpersonal skills.
  • Excellent time‑management and organizational skills.
  • Strong technology skills (use iPad/Laptop to record visit notes).
  • Strong commitment to patient care.
  • Ability to work independently and as part of a team.
  • Valid driver’s license and reliable transportation.
