$40.00/hr - $50.00/hr

• Provision of nursing care; including assessments, treatments, diagnostic procedures, and patient education based on the plan of care and physician’s orders.
• Observe signs and symptoms, reactions to treatment, drug response/interactions, and changes in the patient’s condition; with appropriate follow‑up in care and reporting to supervisor or physician as needed.
• Coordinates care, including care management and discharge planning as needed with the patient/responsible party, physician, other disciplines, and/or internal and external resources.
• Documents accurately, objectively, and timely by regulations and agency policy/procedures.
• Participates in multi-disciplinary team meetings weekly.
• Participates in learning and job‑enhancement activities as directed by the supervisor.
• Participates in On‑Call rotation. One weekday on‑call is required.

• Prior Home Health Experience is preferred.
• OASIS Knowledge is a plus.
• Excellent time management, organizational, and reporting skills.
• Strong communication skills, both written and verbal.
• Great technology skills.
• Current NC or multi‑State RN License.
• Current Driver’s License and automobile liability insurance issued in NC.
