Position Summary
The Client Manager - Nursing (RN) plays a vital role in delivering and overseeing skilled nursing services in clients’ homes. This position ensures individualized, client‑centered care that meets Colorado licensure standards and Home Instead policies. The RN promotes health, manages clinical conditions, provides direct skilled care, and supports unlicensed staff through delegation and supervision.
Key Responsibilities
Clinical Assessment & Care Planning- Conduct initial and ongoing client assessments.
- Collaborate with the interdisciplinary team to create and update individualized Plans of Care.
- Integrate client and family input into care planning decisions.
- Provide direct nursing care including medication management, wound care, catheter care, diabetic care, and other skilled procedures.
- Follow physician’s orders and established care goals.
- Delegate tasks per Colorado Nurse Practice Act and Board of Nursing Rule 13.
- Train, instruct, and supervise unlicensed personnel to ensure competency and safety.
- Maintain accurate and timely clinical documentation in accordance with agency policy.
- Communicate client condition changes to the clinical supervisor and physician.
- Ensure compliance with HIPAA and agency confidentiality standards.
- Educate clients and caregivers about disease management, medications, and home care practices.
- Promote client rights, dignity, and informed choice.
- Participate in supervisory visits and quality management initiatives.
- Comply with infection control and safety protocols.
Qualifications
Minimum Requirements- Active RN license in the State of Colorado.
- Minimum 1 year of nursing experience in home health, community‑based, or related healthcare setting.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Knowledge of CDPHE regulations and nursing scope of practice.
- Strong time management and organizational skills.
- Supervisory or delegation experience with unlicensed personnel.
- Excellent communication, teaching, and problem‑solving abilities.
- Client home visits and occasional office work.
- Travel required throughout service area.
- Possible exposure to bodily fluids and varying home environments.
- Ability to lift up to 25 pounds and assist with client mobility.
- Frequent standing, walking, bending, and reaching.
