- Previous Experience as a Social Worker preferred
- Social Worker License
- Psychosocial assessments/BIMS/PHQ9/Trauma Induced Care
- Attend Care plans/meetings
- MOLST review
- Knowledge of HCPOA/Advanced Directives, PASSAR/
- Work with Interdisciplinary team regarding discharge planning, MDS completion, formulate individualized care plans
- Provide resources PRN
- Advocate for Seniors Aging in Place
- Attend meetings as appropriate
