Outpatient Physical Therapist - Senior Living Visits

BAYADA Home Health Care is currently seeking an experienced Outpatient Physical Therapist (PT) for a full-time opportunity throughout Baltimore County, Maryland.

This position services seniors under their home health benefit within 1-2 communities—home health without the typical commute!

As a home care Outpatient Physical Therapist, you will be a vital part of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to help clients remain at home.

Requirements: At least one year of clinical experience as a licensed Physical Therapist.

• Make outpatient visits to clients in designated geographic areas.
• Perform diagnostic tests, including mobility, range of motion, strength, balance, and safety assessments.
• Develop and implement individualized care plans with manual therapy, gait training, and other interventions.
• Assess and revise care plans regularly, collaborating with nursing staff as needed.
• Educate clients and their families on rehabilitative activities to promote health and independence.
• Document observations and interventions accurately using electronic medical records on a tablet.

• Current Maryland Physical Therapist (PT) license.
• Minimum of one year recent clinical experience as a PT.
• Graduation from an American Physical Therapy Association-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
