Responsibilities Develop individualized treatment plans tailored to each patient's needs
Provide hands-on therapy in the comfort of patients' homes
Collaborate with physicians, nurses, and interdisciplinary team members
Educate patients and families on treatment goals and progress
Make a lasting impact on patients' health, mobility, and well-being
Qualifications
Current and active Physical Therapy License Current CPR Certification Valid driver's license, auto insurance, and reliable transportation
Excellent communication and interpersonal skills
Commitment to patient-centered care Ability to work independently and collaboratively within a team environment
