** Job Summary
** As a Physical Therapist in Home Health, you'll enjoy the freedom and flexibility to balance your personal and professional life while helping patients regain independence and improve their quality of life.
** Responsibilities
* ** Develop individualized treatment plans tailored to each patient's needs
* Provide hands-on therapy in the comfort of patients' homes
* Collaborate with physicians, nurses, and interdisciplinary team members
* Educate patients and families on treatment goals and progress
* Make a lasting impact on patients' health, mobility, and well-being
** Qualifications
* ** Current and active Physical Therapy License
* Current CPR Certification
* Valid driver's license, auto insurance, and reliable transportation
* Excellent communication and interpersonal skills
* Commitment to patient-centered care
* Ability to work independently and collaboratively within a team environment
