Family Medicine APP (Family Medicine) Full-time Cedar City Clinic - Intermountain Health Cedar City, UT

  Up to $7,500 relocation bonus (if applicable).

• Monday - Friday, 8 hour shifts or Tuesday - Friday, 10 hour shifts. Due to demand and access, Fridays are a must.

• Current state license as a Physician Assistant or Advanced Practice Registered Nurse (APRN).
• Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement).
• Eligible for State Controlled Substance License II through V.
• Eligible for Federal DEA Certificate II through V.
• BLS Certification for Healthcare Providers.
• Other certification as required (e.g. ACLS, PALS, TNCC, NRP).

• Master’s degree. Education must be obtained from an accredited institution. Degree will be verified.
