Eligibility:
This position requires a valid driver's license, auto insurance, a good driving record and the ability to pass an extensive background check, including the Dependent Adult and Child Abuse registries, Sex Offender registry and a criminal history check.

Sign-on Bonus: $500.

Location:

Located in the Cedar Rapids, Cedar Valley or Dubuque area. Counties served include Allamakee, Benton, Black Hawk, Bremer, Buchanan, Butler, Cedar, Chickasaw, Clayton, Clinton, Delaware, Dubuque, Fayette, Floyd, Howard, Jackson, Johnson, Jones, Linn, Louisa, Muscatine, Scott, and Winneshiek. Travel outside listed counties may be required.

This is a salaried position with standard Monday–Friday business hours, flexible for occasional evenings or weekends as needed, with on‑call rotation.

• Act as the main contact for interdisciplinary team members and for individuals served in the host home program.
• Work directly with assigned host homes to provide support, assistance, and instruction in areas which facilitate personal growth and independence.
• Facilitate and participate in interviews and evaluations related to the contracting process for independent providers.

• HS Diploma/GED and five years of related experience.
• OR Associates Degree and three years of related experience.
• OR Bachelor’s Degree and one year of related experience.

• Bachelor’s Degree in a human services related field and at least one year of related experience.
• Years of related experience in a supervisory or leadership role is preferred.
