, a 201-bed long-term care skilled nursing community located in Cedar Rapids, IA, is hiring for a Charge Nurse - LPN role.

Location: Heritage Specialty Care, Cedar Rapids, IA

Employment Type: Full Time

Hours: 6pm to 6am

Are you a compassionate Licensed Practical Nurse looking to make a difference? Care Initiatives is seeking a Charge Nurse - LPN to join our team and help deliver exceptional care to residents across our 40+ skilled nursing communities throughout Iowa.

Key Responsibilities
  • Deliver skilled nursing care under the supervision of an RN or DON.
  • Supervise CNAs and other support staff to ensure quality care delivery.
  • Communicate effectively with residents, families, and the care team.
  • Maintain accurate documentation in compliance with federal, state, and company guidelines.
  • Promote a positive, respectful, and compassionate care environment.
Qualifications
  • Current Licensed Practical Nurse (LPN) license in good standing (Iowa or ability to activate).
  • Valid CPR certification.
  • Completion of a State-approved LPN Supervisory Course or willingness to complete within the first 90 days of employment.
  • Knowledge of federal and state long-term care regulations.
  • Strong interpersonal skills with a passion for resident-centered care.
