The Field LPN will provide safe, effective, and compassionate nursing care to patients in their homes under the supervision of an RN. You'll follow the individualized Plan of Care
, ensuring patients' safety, well-being, and satisfaction.

Experience with
Well Sky/Kinnser and home health is a plus.

• Deliver care in accordance with the patient's individualized Plan of Care under RN supervision.
• Provide prompt communication of changes in patient condition/emergencies to the team.
• Perform skilled nursing tasks within LPN scope of practice, which may include:
  • Medication administration
  • Wound care
  • Infusion therapy
  • Transferring & positioning patients
  • Catheter care
• Communicate effectively with patients, families, and interdisciplinary team members.
• Demonstrate empathy, professionalism, and attention to detail while prioritizing patient needs.

• Current LPN license in the State of Florida (required)
• IV Certification (required)
• Strong wound care skills
• 1+ year of home health experience preferred
• Excellent communication, teamwork, and collaboration skills
• Proficiency with electronic health records (EHR)
