The role

We're looking for a Nurse Practitioner or Physician Assistant to join our growing DPC practice on a part-time basis. This isn't a high-volume, insurance-driven clinic, and it isn't a role for someone who needs a rigid protocol for every scenario. Patients come to us specifically because they want something different, and we need a provider who wants to deliver that difference: real relationships, real time, and care that flexes to fit the person in front of them.

About 60% of our current panel arrives at Lineage Wellness after completing Trivida's 4–6 month intensive functional medicine deep-dive. These patients are usually already on a protocol that's working. Your job isn't to start from scratch, it's to maintain what's successful and keep an eye out for the next opportunity to fine-tune it as their goals and labs evolve.
