Transitions Coordinator FTE

As a Transitions Coordinator, you will play a vital role in supporting students with Special Educational Needs and Disabilities (SEND), Education, Health and Care Plans (EHCPs), and other high-needs learners as they move from school into further education. Acting as the key point of contact for students, families, schools, local authorities and external agencies, you will help ensure every learner experiences a smooth, inclusive and successful transition into college.

You will provide person-centred advice, guidance and support while building strong partnerships that enable learners to achieve their full potential., Act as the primary contact for high-needs learners, parents/carers, schools, local authorities and external agencies throughout the transition process.

Develop and maintain strong partnerships with schools, feeder schools and local authorities to support effective transition planning.

Provide impartial information, advice and guidance on suitable learning pathways and support services.

Coordinate transition activities including school visits, learner observations, annual reviews, interviews and enrolment support.

Work collaboratively with curriculum and support teams to ensure learners receive the appropriate support, adjustments and resources they need to succeed.

Monitor learner information, support requirements and EHCP-related processes to ensure compliance with statutory requirements and successful outcomes.

Represent the college at open days, school events and promotional activities, championing inclusive education and learner progression opportunities.

You are a compassionate, organised and proactive professional with a genuine commitment to inclusive education and supporting young people with additional needs. You have experience working with learners with SEND and an understanding of the EHCP framework, enabling you to confidently support students, families and stakeholders throughout the transition journey.

• Experience developing positive relationships with a wide range of stakeholders, including schools, parents/carers and external agencies.
• Knowledge of SEND provision, learner support and progression pathways within education.
• Strong communication, presentation and organisational skills.
• The ability to work collaboratively, manage competing priorities and maintain accurate records.
• A commitment to safeguarding, equality, diversity and inclusion.
• A willingness to travel between campuses and local schools as required.
