We are looking for an enthusiastic, skilled and motivated Rehabilitation Assistant to join our Maximising Independence service across Wandsworth borough to deliver high quality, timely care support with an enablement focus to patients in the community. The Maximising Independence service provides rapid response social care services, in the community, for patients who are at risk of social care breakdown. Without a social care package in place these patients would otherwise be admitted to hospital due to an overriding social care need.

Under the supervision of a clinician, you will work as a rehab assistant providing vital support to therapists (e.g. Physiotherapy and Occupational Therapy) within the team to deliver enabling interventions that support patients with physical, cognitive and psychological needs. This will be directed by the patient's rehabilitation plan to maintain or improve independence in the community.

Responsibilities
  • Assess and manage own caseload of allocated clients referred with specific, predictable presentations under the supervision of a qualified therapist.
  • Provide non‑clinical support to the qualified therapists within the team.
  • Collaborate with colleagues across own and other community teams, acute teams, GPs, social services, and voluntary sector to optimise patients' journey, care, well‑being, and outcomes.
  • Participate in the Trust and local team lone worker policy.
