• Participate in the daily duty rota and efficiently manage incoming calls and referrals.
• Conduct strengths-based assessments under the Care Act to identify individual needs.
• Create and review comprehensive Care Act care and support plans.
• Collaborate with health colleagues to develop Section 117 aftercare plans and execute safe, well-planned discharges from acute or psychiatric hospital settings.
• Act as a Lead Enquiry Officer for safeguarding enquiries, ensuring the safety and well-being of individuals.
• Undertake Carers Assessments and Mental Capacity Act assessments to provide person-centred support.
• Engage actively in team meetings, contributing ideas and fostering a positive team atmosphere.
