The Confederation, Hillingdon CIC is seeking a Social Prescribing Link Worker to join the Primary Care Network multidisciplinary team. You will assess clients, co-produce personalised care plans, and connect people to diverse community services to improve health outcomes.
You will work under GP supervision, manage a caseload, and collaborate with multiple agencies to reduce health inequalities and support mental health where needed. A community-based role with home visits where appropriate.
