Social Work Role

• You will support people to live well in the London Borough of Merton, to maintain their independence, stay at home and improve their safety and quality of life.
• You will help promote prevention by working closely with partners to enable people to live fulfilling and meaningful lives in their homes for longer.
• You will facilitate social connections in communities, add adjustments such as handrails or receive care at home, and empower people to make the best decisions about their care.
• You will work with the Merton‑Transition Team to support young people and their families with additional needs when preparing to move from childhood to adulthood.
• You will collaborate with a range of partners across Children’s and Adult Services, Merton Children’s Teams, SEND service, community health and mental health services.
