Job Overview
We have an exciting opportunity for an experienced social worker to join our multi‑disciplinary team. You will collaborate with colleagues from various disciplines and professional backgrounds to provide holistic palliative care to adults and their families. This role involves working across the human lifespan and with varying care and support needs.
Responsibilities- Assess the social impact of serious illness, end‑of‑life care and bereavement for adults and their families.
- Develop and maintain relationships with adults receiving hospice care and their families to identify unmet care and support needs.
- Collaborate with families to develop plans of support and oversee the implementation and review of those plans.
- Identify positive and potentially harmful risks for adults with palliative care needs living in the community, and formulate strategies to promote positive risk‑taking or mitigate harmful risks.
- Conduct mental capacity assessments and best‑interest decisions.
- Work in partnership with statutory and non‑statutory organisations within a clear framework of legislation and human rights.
- Manage your own caseload with the support of a Senior social worker.
- Provide psychosocial assessments of need, risk assessments and mental capacity assessments as required.
- Adapt work plans and prioritise tasks in a fluid, multidisciplinary environment.
- Negotiate and implement solutions to improve outcomes for people in need of professional support.
- Over 2 years of post‑qualifying social work experience in the UK, including statutory settings.
- Good understanding of health and social care systems and relevant legislative frameworks.
- Experience liaising with statutory services and professionals across disciplines and seniority levels.
- Experience working with loss, grief and the impact of life‑limiting illness.
- Confidence in undertaking psychosocial, risk and mental capacity assessments and best‑interest decisions.
- Strong prioritisation, problem‑solving and systemic practice skills.
- Essential: Experience carrying out social work assessments with adults and/or families using strengths‑based and asset‑based methodologies.
- Desirable: Degree in a relevant subject.
