This role involves working across the human lifespan and with varying care and support needs.

Responsibilities
  • Assess the social impact of serious illness, end‑of‑life care and bereavement for adults and their families.
  • Develop and maintain relationships with adults receiving hospice care and their families to identify unmet care and support needs.
  • Collaborate with families to develop plans of support and oversee the implementation and review of those plans.
  • Identify positive and potentially harmful risks for adults with palliative care needs living in the community, and formulate strategies to promote positive risk‑taking or mitigate harmful risks.
  • Conduct mental capacity assessments and best‑interest decisions.
  • Work in partnership with statutory and non‑statutory organisations within a clear framework of legislation and human rights.
  • Manage your own caseload with the support of a Senior social worker.
  • Provide psychosocial assessments of need, risk assessments and mental capacity assessments as required.
  • Adapt work plans and prioritise tasks in a fluid, multidisciplinary environment.
  • Negotiate and implement solutions to improve outcomes for people in need of professional support.

  • Experience liaising with statutory services and professionals across disciplines and seniority levels.
  • Experience working with loss, grief and the impact of life‑limiting illness.
  • Confidence in undertaking psychosocial, risk and mental capacity assessments and best‑interest decisions.
  • Strong prioritisation, problem‑solving and systemic practice skills.
  • Essential: Experience carrying out social work assessments with adults and/or families using strengths‑based and asset‑based methodologies.
  • Desirable: Degree in a relevant subject.
