Westminster City Council's Adult Social Care team is seeking a Social Worker for the Hospital Discharge Team, based at St Mary’s Hospital and serving the Borough of Westminster. You will assess needs, develop support plans, and contribute to safe, timely discharges while safeguarding through MCA assessments.
You will work five days a week including Saturdays, Sundays and Bank Holidays, joining a multidisciplinary team to support independence and wellbeing in the community.
