Westminster City Council is seeking a Social Worker (weekend working required) to join the Hospital Discharge Team based at St Mary's Hospital and serving the local boroughs. You will undertake assessments of need, develop support plans, carry safeguarding and MCA assessments, and work with carers and other professionals to manage risk while promoting independent living.
