We are seeking a dedicated and compassionate Case Manager to join our team in Centralia, IL. The Case Manager will be responsible for providing support and guidance to individuals and families in need of social services. This includes assessing clients' needs, developing care plans, coordinating services, and advocating for clients' rights and well-being.
Responsibilities:- Conduct assessments to determine clients' needs and develop care plans
- Coordinate and monitor services to ensure clients' needs are met
- Advocate for clients' rights and well-being
- Collaborate with other professionals, such as social workers, healthcare providers, and community organizations
- Maintain accurate and up-to-date client records
- Provide crisis intervention and support as needed
- Bachelor's degree in social work, psychology, or related field
- Previous experience in case management or social services
- Strong communication and interpersonal skills
- Ability to work effectively with diverse populations
- Knowledge of community resources and social service programs
