Position: HIV & Chronic Diseases Care Coordination Team Leader
Location: Centurion, Gauteng
Position
Purpose:
As the HIV & Chronic Diseases Care Coordination Team Leader, you will lead a multidisciplinary team responsible for coordinating care for beneficiaries living with HIV, diabetes, hypertension, cardiovascular disease, mental health conditions, respiratory diseases, and other chronic conditions.
You will oversee clinical and operational quality, mentor and develop the team, manage complex escalations, strengthen provider relationships, and ensure compliance with clinical guidelines and organisational standards. Your leadership will directly contribute to improved health outcomes, enhanced beneficiary experience, and effective risk management.
Experience & Qualifications
:
Registered Nurse (3- or 4‑year qualification) OR health sciences degree — mandatory
3-5 years’ experience in:
- HIV clinical management
- Chronic disease management (e.g., diabetes, hypertension, cardiovascular disease, respiratory disease, mental health)
- Managed care, health risk management, or clinical programme coordination
- Leadership or supervisory experience is advantageous
- Additional HIV or chronic disease-specific training is an advantage
Key Responsibilities:
- Lead and mentor HIV and chronic disease care coordinators
- Oversee care pathways, case management processes, and adherence support across programmes
- Monitor team performance, deliver coaching, and conduct quality assurance reviews
- Analyse programme data and support clinical reporting for HIV and chronic disease outcomes
- Ensure compliance with clinical guidelines, SOPs, regulatory requirements, and scheme rules
- Manage high‑risk or complex clinical escalations requiring advanced judgement
- Engage with healthcare providers, including GPs, HIV specialists, cardiologists, endocrinologists, mental health professionals, and allied teams
- Implement operational improvements that enhance care quality, workflow efficiency, and beneficiary experience
- Support organisational transformation, values‑based leadership, and a collaborative team culture
- Strong clinical judgement across HIV and chronic disease pathways
- Excellent coaching, mentoring, and leadership skills
- High emotional intelligence and patient-centred communication
- Analytical thinking and reporting skills
- Ability to manage complex escalations and support case resolution
- Strong relationship‑building with internal teams and external providers
- Problem-solving ability and attention to accuracy and detail
- Proficiency with clinical systems and electronic documentation
