Renal Care Coordination Team Leader
Location: Centurion, Gauteng
Position
Purpose:
As the Renal Care Coordination Team Leader, you will lead a specialised team of clinicians who provide comprehensive support to beneficiaries with chronic kidney disease (CKD), End‑Stage Kidney Disease (ESKD), and those undergoing dialysis or post‑transplant care. You will play a pivotal role in managing clinical and financial risk, improving patient outcomes, enhancing treatment adherence, and ensuring smooth coordination between nephrologists, dialysis centres, transplant units, and internal stakeholders.
Experience &
Qualifications:
Registered Nurse (3- or 4‑year qualification) OR health sciences degree — mandatory
3 to 5 years’ experience in:
- Renal nursing or nephrology case management
- Dialysis (hemodialysis or peritoneal dialysis) pathways
- Managed care, health risk management, or clinical customer service
- Previous experience managing or supervising a clinical team (advantageous)
- Strong understanding of CKD staging, dialysis modalities, transplant processes, and chronic renal disease management.
Key Responsibilities:
- Lead and mentor the renal care coordination team to deliver high‑quality service
- Oversee renal case management, including CKD monitoring, dialysis authorisation, transplant readiness, and patient follow‑up
- Analyse renal programme data, clinical outcomes, and utilisation trends
- Ensure compliance with renal care guidelines, managed care rules, and regulatory frameworks
- Build strong relationships with nephrologists, dialysis providers, transplant units, and multidisciplinary teams
- Support beneficiaries with education, navigation, and adherence to renal care pathways
- Improve workflows, contribute to SOPs, and streamline renal care processes
- Manage team performance, development, engagement, and capacity
- Address escalations and ensure exceptional beneficiary and stakeholder experiences.
- Strong clinical judgement and renal‑specific risk assessment abilities
- Excellent communication and business writing skills
- Analytical and problem‑solving ability
- High attention to detail and accuracy
- Strong relationship‑building and stakeholder engagement skills
- Computer literacy and comfort using clinical systems
- Leadership skills with a focus on coaching and team development
- Empathy, patient‑centred communication, and resilience.
