Position:
Care Coordinators – Chronic Diseases
Location: Centurion, Gauteng
Purpose
:
As a Chronic Diseases Care Coordinator, you will support beneficiaries living with conditions such as diabetes, hypertension, cardiovascular disease, asthma/COPD, epilepsy, autoimmune disorders, mental health conditions, and other long‑term illnesses.
You will play a central role in care coordination, beneficiary education, lifestyle counselling, clinical risk identification, and navigating beneficiaries through chronic disease pathways. The role also includes the management of clinical and operational queries from beneficiaries, healthcare providers, and internal teams.
Experience &
Qualification:
- Additional training in chronic disease management or lifestyle counselling is advantageous
- Strong knowledge of the SA healthcare market, PMBs, and managed care principles.
- 2 to 5 years’ experience in:
- Chronic disease management or primary healthcare
- Managed care, health risk management, or care coordination
- Clinical customer service or healthcare administration
- Experience working within the medical scheme or healthcare administration environment is an advantage.
Key Responsibilities:
- Conduct clinical assessments and develop personalised care plans
- Support beneficiaries with ongoing disease management, medication adherence, and lifestyle modification
- Monitor clinical indicators, pathology results, and symptom trends
- Identify high‑risk cases and elevate appropriately
- Provide education on nutrition, physical activity, stress management, and disease‑specific health literacy
- Collaborate with GPs, specialists, psychologists, and allied healthcare providers
- Manage beneficiary and provider queries within agreed turnaround times
- Investigate queries through clinical record review, pathology analysis, and treatment history
- Document all interactions accurately in care management systems
- Provide compassionate, patient‑centred communication and support
- Strong knowledge of chronic disease clinical pathways
- Excellent communication, counselling, and education skills
- Clinical risk assessment ability
- Emotional resilience and a patient‑centred approach
- High accuracy and attention to detail in documentation
- Strong analytical and problem‑solving capabilities
- Ability to work with multidisciplinary teams
- Computer literacy and proficiency with clinical systems
