Case Manager - Community Outreach

About the Role

Our client is looking for a proactive and empathetic Case Manager to lead their Community Outreach initiatives based in Centurion . This role is crucial for connecting vulnerable individuals and families with essential support services, fostering independence, and improving their quality of life. You will be responsible for managing a caseload, conducting home visits, coordinating services with various stakeholders, and ensuring clients receive the necessary assistance to overcome challenges.

• Manage a diverse caseload, providing comprehensive case management services.
• Conduct client assessments to identify needs, strengths, and barriers.
• Develop, implement, and monitor individualized service plans for clients.
• Facilitate referrals to and coordinate services with other community agencies and healthcare providers.
• Conduct regular home visits and follow-ups to ensure client engagement and progress.
• Maintain accurate and timely case documentation in compliance with agency standards.

• Degree in Social Work, Psychology, or a related social science field.
• At least 2 years of experience in case management or community-based services.
• Valid driver's license and reliable access to a vehicle for home visits.
• Strong knowledge of community resources and referral pathways.
• Excellent organizational, problem-solving, and communication skills .
• Ability to work effectively with diverse populations and in challenging situations.
