Our client is seeking an experienced and dedicated Social Care Officer to serve the Centurion community. This position involves providing crucial support and assistance to vulnerable individuals and families, ensuring their safety, well-being, and access to necessary resources. You will be a key point of contact, building trust and rapport while navigating complex social issues. The role requires a proactive approach, excellent interpersonal skills, and a deep understanding of social welfare systems.
Working within a supportive team, you will contribute to positive outcomes and foster resilience within the community. This is an on-site role demanding consistent presence and engagement within the local area.
- Conduct thorough assessments of client needs, including risk assessments and care plan development.
- Deliver direct social care services, offering practical support, counseling, and advocacy.
- Collaborate with families, caregivers, and relevant agencies to coordinate care and support services.
- Monitor client progress, regularly review care plans, and adjust interventions as needed.
- Maintain detailed and confidential case notes and reports in compliance with organizational policies.
- Organize and facilitate community-based programs and support groups.
- Degree in Social Work, Psychology, or a relevant social science field.
- A minimum of 3 years of professional experience in social work or a closely related role.
- In-depth knowledge of South African social welfare legislation and services.
- Exceptional problem-solving abilities and crisis management skills.
- Strong reporting and documentation capabilities.
- Ability to build rapport and work effectively with diverse client populations.
