A healthcare service provider in California is seeking a Hospice Intake / Care Coordinator as the first point of contact for hospice services. This role includes coordinating the intake process, verifying patient eligibility and insurance, and facilitating communication between clinical and administrative teams. Ideal candidates should have strong communication skills, either an LVN license in CA or MA, and familiarity with insurance and EMR systems.

Preference is given to those with hospice experience.
