Care Manager Registered Nurse - Field; Champaign, Vermillion, Ford Counties, IL

The Care Manager Registered Nurse will utilize critical thinking and judgment to facilitate appropriate healthcare outcomes for members, providing assistance with appointment scheduling and accessing benefits. • Pay: $66,575 - $142,576/year
• Location: Champaign, IL

• Schedule: Full-time
• Travel: Up to 50-75% travel required

• Collaborate and inform the case management process.
• Facilitate appropriate healthcare outcomes for members.
• Assist with appointment scheduling.

Education:

• Associate's degree required, bachelor's preferred

Experience:

• Minimum 3-5 years clinical practical experience
• 2-3 years Care Management, discharge planning and/or home health care coordination experience preferred

• Active and unencumbered Registered Nurse license in the state of Illinois required
