• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
• Successful completion of an accredited residency program in Family Medicine.

• Board Certified in Family Medicine.
• Current or eligible for an active Arizona medical license.
• BLS and ACLS certifications preferred.

• Demonstrated excellence in clinical evaluation and diagnostic skills, with a firm commitment to preventive healthcare.
