We are looking for a compassionate Family Nurse Practitioner (FNP) who is passionate about building meaningful, long‑term relationships with patients. In this role, you’ll provide comprehensive care to individuals and families across all ages, focusing on prevention, health education, and empowering patients to take an active role in their well‑being.

• Deliver high-quality primary care through assessments, diagnoses, and treatment.
• Partner with patients to create personalized care plans that reflect their goals and values.
• Educate and engage patients on preventive health, wellness, and chronic condition management.
• Collaborate with a supportive team of healthcare professionals to ensure holistic, patient-centered care.
• Foster trust, empathy, and open communication with patients and families.

• Certified Family Nurse Practitioner (AANP or ANCC) with an active state license.
• A strong commitment to patient care, engagement, and education.
• Excellent communication and listening skills.
• A compassionate, approachable, and collaborative attitude.
