* This is a full-time position offering a Monday-Friday schedule, providing service throughout the East Valley (including Mesa, Chandler, Gilbert)!

As a Licensed Practical Nurse (LPN), you'll provide hands-on patient care under the guidance of a Registered Nurse, ensuring comfort, safety, and positive outcomes for patients in their homes. • Deliver compassionate, direct patient care in accordance with state regulations
• Implement care plans developed by the RN and document progress accurately
• Monitor patient conditions and communicate observations to the RN and physician
• Collaborate with the care team to ensure seamless, high-quality service
• Participate in professional development and team meetings

• Currently licensed as a Licensed Practical Nurse (LPN) in the State of Arizona
• 3 years of nursing experience preferred (Home Health or Med Surg a plus)
• Current CPR certification
• Valid driver's license, auto insurance, and reliable transportation
