We have patients in Chandler/Mesa/Tempe/Scottsdale and other locations in the East Valley.

What You'll Do

• Provide direct patient care in the home
• Monitor patient condition and report changes
• Administer medications and treatments as ordered
• Support individualized care plans
• Document care and communicate with the healthcare team

Requirements

Required:

• Active LPN license
• Current BLS certification

Preferred:

• 1+ year of nursing experience

Additional Details
